Improving Lives Initiative - Community Connectors

The Community Connectors will signpost people to community supports provided by Community and Voluntary Sector (CVS) Organisations and, where appropriate, link to statutory services such as Home Assessment Teams and Integrated Rehabilitation Teams.

Our community connectors can offer:

Practical support, such as form filling
Linking into community groups or activities
Information on benefits/funds/initiatives
Emotional support
Signposting and introductions to other helpful services.
